In order to facilitate success with your new food plan, Heather will also be your personal shopper!
Complete the "Contact Info" form below, then select the items on the shopping list and note the quantity, amount, and/or weight for each. If you have any specific requests, please put them next to the particular item or at the end of the list where it says "other/specifics".
When you are done, hit "Submit" and Heather will contact you within 24 hours by phone to confirm your shopping list.
Shopping will be completed within 48 hours of request, and will be picked up at one of several locations:
